Recent Success Stories
GP Super Practice
We were contacted to lead the establishment of a new GP Super Practice from twelve constituent practices. In a period of five months we developed and implemented a detailed programme plan to bring the twelve together under a new partnership agreement, with finance and payroll systems and a business strategy for the next two years.
Integrated Healthcare Programme
We were commissioned to lead the establishment of neighbourhood teams to deliver place based care across a countywide footprint. Over eighteen months we worked with all health sectors, local authority and the voluntary sector to define the model specification, ways of working and desired outcomes. We successfully established a countywide network of teams, who were operational and moving forward to better integrate care for patients
Medicines Management Implementation
We were approached by a company to deliver a CCG-wide guidance on behalf of their medicines management customers. Over a six month period, we worked with their customers and implemented the guidance across fifty GP practices. The CCG achieved their desired outcomes on time and the company were able to deploy their team elsewhere until project completion and handover.
